Colposcopy And LLETZ

Understanding and Treating Abnormal Cervical Screenings

Obstetrics & Gynecology > Colposcopy And LLETZ

A gynaecologist telling you that you may need a colposcopy done can evoke a storm of feelings, such as confusion, worry, and maybe even fear. If you have had an abnormal Pap test and your doctor tells you that something is not right, it is very normal to feel overwhelmed.

It is important to remember that a colposcopy allows your doctor to take a closer look, to see what is going on, and to determine if any treatment is needed.

Colposcopy

What is a Colposcopy?

A colposcopy is a diagnostic test which allows your gynaecologist to look at the cervix, vagina, and vulva in detail through a special instrument known as a colposcope. It is usually done when you have had atypical results from a Pap test or a human papillomavirus test.

This is a painless test that is used to identify precancerous and, in some cases, cancer cells. While performing the test, if the colposcopist finds anything suspicious, a small biopsy is taken during the procedure itself and they send it for further examination.

What is LLETZ?

Large Loop Excision of the Transformation Zone is a small-scale surgical procedure that removes abnormal or precancerous cells from the cervix. It is also performed when dysplasia (growth of abnormal cells) is identified during a colposcopy.

The LLETZ procedure removes the affected tissue in the cervix and is a safe and effective method that prevents the progression of abnormal cells into cervical cancer.

In some gynaecology cases or settings, LLETZ may also go by the term LEEP (Loop Electrosurgical Excision Procedure).

When Are These Procedures Recommended?

Following routine screening, your doctor may recommend a colposcopy test and, if necessary, LLETZ if:

  • Your pap smear or pap tests were not entirely normal.
  • You are reported to have tested positive for high-risk HPV strains.
  • You have unusual vaginal bleeding or discharge.
  • You have visible abnormalities on your cervix.
  • Your symptoms indicate vulvovaginitis or genital warts.

How is The Colposcopy Procedure Done?

  • You must lie down on your back, but your feet will be on stirrups.
  • The gynaecologist doctor will insert a speculum into the vagina to view the cervix.
  • Then, a colposcope (an instrument which is like a magnifying tool) will be used to examine the cervix as it provides a better view.
  • The colposcopy doctor will then apply a solution that is acid or vinegar-based on the cervix that highlights atypical cells.
  • If any unusual or suspicious cells are found, a colposcopy biopsy will be done to take a tissue sample, and the sample will then be sent to the lab for further testing.

The colposcopy operation procedure can last anywhere between 10 to 30 minutes. Most women report mild symptoms, which include pain similar to that of a Pap test, but nothing that is uncomfortable.

How is The LLETZ Procedure Done?

  • You must lie down on your back with your feet on stirrups.
  • Cervical anaesthesia will be administered to reduce pain.
  • A colposcope is used to get a clear, magnified view of the cervix and perform the procedure.
  • The colposcopist will then use a thin wire loop that carries an electric current for the precise removal of abnormal cells.
  • The extracted tissues are sent in for more in-depth analysis.
  • After the LLETZ procedure is completed, you may feel cramping or experience spotting, but they are mild and don’t last for very long.

The LLETZ gynaecology procedure also lasts between 15 and 30 minutes. Recovery is usually quick, and the majority of women return to their normal activities within a few days.

FAQs

Yes, most people may go home and drive after a colposcopy, which is fine if no biopsy was taken. However, if a biopsy has been done and you are experiencing cramps, lightheadedness, or feeling unwell, we recommend arranging for a friend or family member to give you a lift.

Yes, it is possible to get pregnant after having a colposcopy done. It is a diagnostic procedure that examines the uterus, and the procedure itself does not affect anything, even if a biopsy is done.

It is better to not be on your period when getting the procedure done, as bleeding can obscure the view and affect the diagnosis. However, if you have light spotting, you can get the procedure done, but it is best to discuss it with the doctor who will be conducting the procedure.

No, that’s not the same as a biopsy. A colposcopy is a procedure done to only check the cervix and look for anything unusual. A biopsy may be performed at the time of the procedure if anything unusual is found.

A colposcopy procedure itself does not diagnose cancer but is done to detect abnormal cells. If anything unusual is present, a biopsy is taken and the sample is sent for further diagnosis to determine if the cells are cancerous or precancerous.
