Endometrial Sampling

Helping you understand what your uterus is trying to tell you.

Obstetrics & Gynecology >  Endometrial Sampling

Dealing with missed periods you have been anticipating, or worried about heavy menstrual flow? Or is unexpected spotting disrupting your daily routine? If yes, these symptoms can be very confusing and stressful. Moreover, when medications and lifestyle changes don’t do the trick, it is only natural to want more concrete answers.

At that point, your gynecologist doctor may recommend endometrial sampling to take a closer look at the lining of your uterus and determine what is really happening.

Endometrial Sampling

What Is Endometrial Sampling?

Endometrial sampling is the process of removing small tissue from the uterine lining (the endometrium) for examination. The tissue is viewed under a microscope to determine the cause of abnormal symptoms.

It looks at the lining for any above-average endometrial thickness, also for polyps or signs of inflammation. This test also eliminates or identifies more serious conditions like endometrial hyperplasia or endometrial cancer.

Why Is An Endometrial Test Recommended?

You may have noticed that your periods are irregular or very heavy, or that you are spotting when you shouldn’t. Or maybe you are postmenopausal and see some bleeding that is out of the ordinary, or you are on hormone therapy and have other unexpected symptoms. You may be trying to get pregnant but are having trouble conceiving

This test is recommended for such situations. It can also be used to check for signs of infection and inflammation in the uterus. While these could also be endometrial cancer symptoms, they could signal other health conditions as well, which is what this test can help identify.

How to Prepare for the endometrial test?

Here is a list of things you can do to feel at ease and ready for the endometrial test:

  • For at least 24 to 48 hours before the test, do not use tampons, engage in sexual intercourse, or douching. This will help to keep things clean, which will provide better results.
  • Your gynecologist doctor may set the test during a certain time in your menstrual cycle, at a stage that is determined to be the best for the evaluation.
  • Report to your healthcare provider what medication you are on; also let them know if you are on blood thinners or hormone regimens.

What Happens During the Procedure?

The process lasts for about 15-20 minutes, which goes like this:

  • A speculum, the same tool used during a Pap smear, is placed to gently open the vagina so the doctor can see your uterus opening.
  • A tenaculum is used to hold the cervix still, and a small flexible suction tube is inserted to take a sample of your endometrial tissue.
  • The tissue is vacuumed out, which may cause a sensation of mild cramping or pain, which in most cases is very brief. Most women describe the sensation as a pinch or a sudden pressure.
  • The removed endometrial tissue is then sent for further analysis.

What Can the Results Tell You?

Here’s what the results from the endometrium sampling can provide:

  • Endometrial Hyperplasia: This is when the endometrial lining is thicker than usual, which can cause heavy bleeding. It is usually treatable with medication or minor procedures.
  • Endometrial Polyps: These are small growths that may cause spotting or bleeding, and they may need to be removed if they lead to discomfort or any other issues.
  • Endometrial Cancer: It happens when cells in the lining grow out of control. It usually grows slowly and often shows early signs like unusual bleeding, making it easier to detect and treat early. An endometrial ultrasound may be needed to confirm the presence of cancer.

A normal report indicates that the endometrium lining has normal thickness, and your symptoms may be due to more minor issues.

Are There Any Risks?

Most women don’t report any issues; that said, you may see:

  • Gentle cramping during or right after the procedure.
  • Spotting for a couple of days.
  • Very rarely, an infection or injury could occur.

Treatment Options Based on Your Results

Based on what your doctor determines:

  • Endometrial thickness treatment: Medication for hormonal balance or to manage bleeding.
  • Endometrial hyperplasia treatment: Outpatient procedures include the removal of endometrial polyps or thickened endometrial tissue and hormone therapy medication called progestin therapy.
  • Endometrial cancer treatment: If cancer is diagnosed, a personalized treatment plan may include surgery or a therapy to ensure the removal of cancer cells.

FAQs

Most women report a light cramping or pressure, kind of a quick pinch. It’s brief, and your doctor can help you feel comfortable throughout.

Usually, we see results between 5-7 days. Your doctor will explain the results and discuss what to do next.

We don’t recommend this test for pregnant women because it requires sampling of the uterine lining.

It can detect abnormalities in the endometrial lining but may need to be combined with other tests, like an endometrium ultrasound, and additional blood tests for a thorough diagnosis.
